Call us or set up your own free of charge initial intake appointment on the calendar by first choosing your location below. Georgia photo ID required. When using the calendar, choose your date, time, and location. For this initial intake appointment, which includes a medical assessment, allow one hour.  Our medical staff will determine what other medical appointments you can make according to your assessment.

If you are calling outside our normal business hours, please just leave your contact information and we will call you back on our next business day.

If you are an established patient and wish to make a follow-up visit, you may do so at the educational center on the calendar.
